The Diabetes Prevention and Control Alliance has developed two proven programs—one for diabetes prevention, another for diabetes control—based on proven programs.
For many participants, these programs are available at no additional cost, as part of their health plan. Contact us*, your employer or health plan provider today to see if you are covered.
Diabetes Prevention Program
79 million Americans are estimated to have prediabetes, which means they have a high risk of developing diabetes.
Preventing diabetes doesn't require major lifestyle changes. Once you understand the risk factors, get yourself tested, and begin to eat healthier and get active, you can be well on your way.
Are You at Risk?
Take our 6-step diabetes self-assessment risk quiz to find out if you may be at risk for type 2 diabetes. If you are, enrolling in our Diabetes Prevention Program can help you say, "NOT ME" to diabetes.
Diabetes Control Program
If you have already been diagnosed with diabetes, we can help. Our Diabetes Control Program can teach you how to make small life changes with big benefits.
We can show you how to better control your type 2 diabetes through regularly scheduled, private, in-depth consultations with a specially-trained pharmacist.
You can get help to:
  • Manage your medications
  • Improve blood glucose, cholesterol, blood pressure and body mass index
And, this will all be coordinated with your primary care physician.
